Body Dysmorphic Disorder

Have you ever wished that you had a nicer nose? A more dashing smile, perhaps like your favorite actor? Or smooth silky hair like your favorite actress? Rest assured, these are all normal human thoughts. However, some individuals do cross the line of common human vanity with a psychological disorder known as body dysmorphic disorder. It is believed that there is a strong link between individuals who display signs of cosmetic surgery addiction and body dysmorphic disorder.

Knowing the exact number of population suffering from body dysmorphic disorder is difficult as many never come forward to consult their health care provider, which leads to them never being formally diagnosed. However, it is estimated that as high as one in one hundred individuals suffer from body dysmorphic disorder. Given how common yet the high number of undiagnosed cases that are present, it is absolutely vital to bring attention to body dysmorphic disorder.

Body dysmorphic disorder usually begins in late teens and early adulthood. Individuals are usually preoccupied with an imagined or a slight physical defect unnoticeable by others. This perceived physical defect causes distress to the point that it impairs their ability to function socially and in their occupational setting. The distress can be on any part of the body, though facial features tend to be the most common focus of their anxiety. Moreover, individuals suffering from body dysmorphic disorder tend to show certain elements of obsessive-compulsive disorder as well as anxiety in their behavior routine. They repeatedly obsess over physical features, which subsequently leads to the building up of anxiety. The compulsion part is their attempt to reduce their anxiety by satisfying their obsession. In the case of body dysmorphic disorder, compulsive behavior can be a simple gesture in asking friends and family how their body part looks or visiting their doctor to discuss their body feature.

For a better understanding, let’s have a look at this scenario:

Ms. Jones is a twenty-one-year-old student at a local university. She thinks that she has a big nose and often asks her family members what they think of her nose. Her family always gives the honest response that her nose does not look big to them at all. She often appears late for her morning class due to the time spent in front of the mirror, fixing her hair and makeup, with the intent to take attention away from her nose. She wastes so much time on this routine each morning that she neglects important things, such as fixing breakfast and checking homework for class.

Once she finally waltzes in late to her first class of the day, she immediately asks her classmate Kelly, how does her nose look? Does it look big, and how does her nose compare to other classmates? Kelly knows the routine all too well. She always answers truthfully that Ms. Jones is a very beautiful girl and that she sees nothing wrong with her nose.

This routine is beginning to have an effect on her socially. She often does not sit with her friends at lunchtime as she wastes away time looking at her nose in the bathroom mirror. On the weekends she has been canceling plans to go to dinner and see a movie with her best friend Kelly in fear of being anxious of people looking at her nose in public.

Today Ms. Jones visits the school’s health department. She confesses to the school doctor that she has a test today but did not take the exam as she is unable to concentrate. Her thoughts are always fixated on her big nose. She continues to elaborate to him her thought process over the past few months. She further explains that over the last two weeks she has consulted with three plastic surgeons and four primary care doctors, including her regular doctor about her nose. She confides to the school doctor that she was shocked when all of the doctors she visited with unanimously expressed to her that her nose looks just fine and that they do not see any physical defects. She is anxious that no one can solve her problem.

Her physical examination proves normal other than the fact that she keeps fidgeting with her hands and picking at the skin of her fingers. This behavior indicates to the doctor that Ms. Jones’ anxiety level is quite high.

With her history of visiting many doctors to complain about her physical appearance coupled with her anxiety about her physical feature to the point that it impairs her socially and academically, the school doctor has a very strong suspicion that Ms. Jones suffers from a psychological disorder termed body dysmorphic disorder. The school doctor will refer her to see a psychiatrist and ask her to return to see him again next week at the school’s health department.

The treatment for body dysmorphic disorder begins with frequent follow up appointments with a psychiatrist, and the patient’s primary care provider in order to decrease the patient’s anxiety and to keep track of the patient’s progress. Individual psychotherapy provided by the psychiatrist is essential as understanding the root of the fixture on the body part is the key. Whether it is a life event that has affected the patient’s self esteem or a prior psychological trauma, understanding the patient’s subconscious conflict, the psychiatrist can begin to give cognitive and behavior therapy and install understanding and coping skills to the patient, preventing depression and the relapse of recurrent thoughts.

On the pharmacotherapy side, SSRIs class of medication such as olanzapine (Zyprexa) helps to decrease anxiety resulting in decrease in the patient’s compulsive behavior.

Last but certainly not least, family therapy. There is nothing more important than educating family members on body dysmorphic disorder, as understanding and support from family is the most effective initial step towards recovery.

ผลการตรวจร่างกายของเธอไม่มีอะไรผิดปกตินอนจากการแสดออกในการหยิกแกะนิ้วและดึงกับมือของเธอ พฤติกรรมเช่นนี้แสดงให้เห็นว่า น.ส jones มีระดับความกังวลสูงมาก จากประวัติการแพทย์ของเธอซึ่งไปพบแพทย์มาแล้วหลายคน เพื่อที่จะบอกกล่าวถึงความผิดปกติของร่างกายของเธอ ประกอบกับความกังวลใจเกี่ยวกับรูปโฉมของเธอ จนทำให้เกิดความเสียหายในด้านการเรียนและการเข้าสังคม แพทย์ประจำโรงเรียนมีความสงสัยว่า น.ส jonesจะมีอาการผิดปกติทางจิตที่เรียกว่า Body dysmorphic disorder หรือโรคดูถูกตัวเอง และแนะนำให้เธอไปหาจิตแพทย์และขอให้เธอกลับมาพบหมอประจำโรงเรียนอีกครั้งที่แผนกแพทย์ของโรงเรียนในอาทิตย์หน้า

การรักษาโรค body dysmorphic disorder หรือโรคดูถูกตัวเอง เริ่มต้นด้วยการไปพบจิตแพทย์และแพทย์ประจำครอบครัวเป็นประจำเพื่อที่จะลดความกังวลของผู้ป่วยและติดตามผลของการรักษา การทำจิตภาพบำบัดส่วนบุคคลซึ่งกระทำโดยจิตแพทย์ มีความจำเป็นที่จะต้องเข้าใจถึงสิ่งที่อยู่ในใต้จิตสำนึกส่วนลึกที่อยู่ในตัวของเธออันจะเป็นหัวใจสำคัญในการรักษาโรคนี้ ไม่ว่าเหตุการณ์ในชีวิตที่ผ่านมาจะมีผลต่อความหลงตัวเองของคนไข้ หรือมีความชอกช้ำทางจิต,ความเข้าใจในความขัดแย้งในจิตใต้สำนึกของผู้ป่วย ,จิตแพทย์สามารถเริ่มให้ความรู้ความเข้าใจและบำบัดปรับเปลี่ยนพฤติกรรม รวมถึงทำให้เกิดความเข้าใจทัศนคติใหม่ๆ เพื่อป้องกันอาการซึมเศร้า และทำให้ความคิดที่ผิดปกติไม่ให้เกิดขึ้นมาอีก

ทางด้านการรักษาทางยา ,ยา ในกลุ่ม SSRIs เช่น olanzapine( Zyprexa) จะช่วยลดความกังวลซึ่งจะมีผลทำให้คนไข้ลดพฤติกรรมที่เคยกระทำมา สิ่งสุดท้ายคือการช่วยเหลือบำบัดจากครอบครัว ไม่มีอะไรที่จะสำคัญมากกว่าการให้ความรู้แก่สมาชิกในครอบครัวเกี่ยวกับเรื่อง body dysmorphic disorder หรือโรคดูถูกตัวเอง เพราะความเข้าใจและการสนับสนุนจากครอบครัวเป็นสิ่งที่สำคัญและให้ผลสัมฤทธิ์มากที่สุดเป็นสิ่งแรกที่จะให้ผู้ป่วยหายจากอาการดังกล่าว
