Migraine Headache

Greetings dear brothers and sisters,

Every week I receive many interesting medical questions in my inbox. From now on I will be posting some of the questions here so we can all share a constructive discussion.

I understand that anonymity is very important for everyone so I will refer to the questioner as Mr. X, Mrs. Y, or Miss Z, etc.

This week I received a message from Miss X who said she has been experiencing bouts of headaches, insomnia and a clock-like ticking sound in her head. “คุณหมอคะขอรบกวนนะคะ อยากรบกวนสอบถามเกี่ยวกับอาการ หากได้ยินเสียงเหมือนเสียงนาฬิกาอยู่ในสมอง ร่วมกับอาการนอนไม่ไม่หลับ ควรไปหาหมอทางสมอง หรือหมอทางจิตเวช คะ ขอโทษที่รบกวนนะคะคุณหมอ”

She asked for suggestions. I replied to her asking further details about the sound she hears in her head, as I was trying to get a differential diagnosis. (People who take a lot of aspirin experience ringing in their ears – the first sign of aspirin toxicity, also people who have tumors on their cranial nerve 8 usually experience ringing in their ears, and people with Meniere’s syndrome also experience ringing in their ears. These are some examples of differentials with a ringing sound and a headache.) I asked her if the headache was on one side or on both sides of her head. Beyond my assessment I advised her to immediately go see her physician to perform a full neurological work-up, which includes a CT scan.

Yesterday Miss X updated me that her physician diagnosed her with migraine headaches. There are different kinds of headaches: tension, migraine, cluster, etc. Migraines usually present as a unilateral headache that can be triggered by foods such as cheese, excessive caffeine, menstruation, and many other variables. Patient can experience an aura of bright lights, loss of vision or a sense of abnormal smell before the headache sets in.

I suggested a discussion of prophylactic measures to Miss X that she could consult with her physician, if her migraine becomes more frequent. I shared with her that beta-blockers such as propranolol could prevent future episodes of migraines. Miss X says that she was surprised to hear this, as beta-blockers are blood pressure medications. Beta-blockers are blood pressure medications that work by slowing down the contractility of the heart. They also can serve other functions, such as treating essential tremors and migraine prophylaxis. Furthermore, I shared with her that medicines such as triptans (usually sumatriptan) are considered abortive therapy, this is if she comes to the emergency room with severe migraines, the ER physicians would promptly administer this medication.

ขอโทษที่รบกวนนะคะคุณหมอ ผู้ป่วยได้ขอคำแนะนำ แต่ผมได้อธิบายให้แบบละเอียดว่าเสียงที่เขาได้ยินในหัวนั้น ผมพยายามจะวินิจฉัยและนึกถึงสาเหตุต่างๆ คนที่ใช้ยาแอสไพรินมักจะมีอาการมีเสี่ยงสั่น(คล้ายเสียงนาฬิกาปลุกแบบโบราณ)ในหู ซึ่งถือว่าเป็นอาการข้างเคียงของผู้ที่ได้รับพิษจากยาแอสไพริน รวมถึงคนที่มีเนื้องอกในประสาทหูก็มักจะได้ยินเสียงสั่น ส่วนที่ที่เป็นโรคเวียนศรีษะก็มักจะมีอาการคล้ายกันนี้ ผมได้ถามเธอว่าเธอปวดศรีษะข้างเดียวหรือสองข้าง นอกจากนี้ผมยังแนะนำให้เธอไปตรวจกับแพทย์ผู้เชี่ยวชาญเฉพาะสาขาให้ตรวจรักษาแบบละเอียดรวมถึงการตรวจสมองแบบ CT (เอกซเรย์คอมพิวเตอร์)

เมื่อวานนี้นาง ก ได้ มารายงานความคืบหน้าผลการตรวจวินิจฉัยไมเกรน การปวดหัวมีหลายแบบด้วยกัน เช่น เครียด ไมเกรน ปวดเหมือนศรีษะถูกบีบ การปวดศรีษะแบบไมเกรนมักจะปวดข้างใดข้างหนึ่ง โดยมากแล้วจะถูกกระตุ้นด้วยอาหารบางอย่าง เช่น ชีส ดื่มเครื่องดื่มที่มีส่วนผสมของคาเฟอีนเยอะไป ประจำเดือน และตัวแปลอื่นๆ ทั้งนี้ทั้งนั้นผู้ป่วยอาจจะมีอาการเห็นแสงสว่าง ความสามารถในการเห็นลดลง หรือได้กลิ่นแปลกๆก่อนปวดศรีษะ

ผมได้แนะนำให้ นาง ก ไปตรวจเพื่อป้องกันไว้ก่อน ดังนั้น นาง ก ควรไปปรึกษาแพทย์ส่วนตัว ถ้าเขายังปวดศรีษะข้างเดียวอย่างต่อเนื่องแล้วล่ะก็ ยา บีต้า บล็อคเกอร์ เช่น propranolol สามารถ ป้องกันไมเกรนที่อาจจะเกิดขึ้นได้ บีต้า บล็อคเกอร์เป็นยาความดันเลือดทำหน้าที่การเต้นของหัวใจและช่วยรักษาอาการปวดศรีษะข้างเดียว นอกจากนี้ผมยังบอกนาง ก ว่า ยาในกลุ่มtriptans หรือมีอีกชื่อหนึ่งคือ sumatriptan ยาตัวนี้จะไปหยุดไมเกรนไม่ให้มีผลต่อผู้ป่วย ถ้าหากนาง ก มีอาการปวดไมเกรนแบบรุนแรงมากจนถึงกับต้องเข้าห้องฉุกเฉินแล้วแพทย์จะให้ยานี้ทันที
