Dietary Fiber: Key to Healthy Bowel

Mr. Smith is a sixty-seven-year-old man who is an owner of a ranch located in Southern Australia. Today, at the urging of his wife, he visits the local clinic with the complaint of abdominal discomfort.

It has been well over ten years since Mr. Smith has seen a healthcare provider for a checkup. He often boasts to his wife and fellow cattlemen of his good health by pointing out that he has been running a successful ranch year in and year out without any sign of slowing down. Most importantly, he loves the routine and the lifestyle of a rancher.

As the clinic does not have Mr. Smith’s medical record on file as this is his first visit, the doctor would like to begin by exploring his medical history as well as meticulously inquire into his current symptoms.

Mr. Smith begins by explaining to the doctor that he has always perceived himself to be a healthy individual. In spite of his belief, he has noticed that for the duration of the past ten years he has experienced some digestion problems, which seem to be progressing worse. Mr. Smith elaborates to the doctor that at first, it began with occasional constipation which he was able to handle with the over the counter stool softener.

However, around three years ago, he discovered that he needed to spend longer periods of time in the bathroom, straining during bowel movements in order to “get the deed done.” Presently, in addition to the long and uncomfortable straining episodes when passing stool, Mr. Smith explains to the doctor that he can feel his stomach feeling bloaty with an abdominal cramp on his left lower side that comes and go.

Yesterday while cleaning the house, his wife discovered blood in the toilet bowl. She confronted him about his health with a gentle reminder that when they took their wedding vows they promised to be honest with one another. The disappointment on his wife’s face prompted Mr. Smith’s spontaneous visit to the health clinic today for the first time in over ten years.

When the doctor asks Mr. Smith to elaborate more about the blood in the toilet bowl, he reluctantly confesses that for the past year, he occasionally passes blood along with his stool (hematochezia). He seems to validate the fact that he hasn’t sought medical help by pointing out that while the bleeding appears bright red and can certainly be nerve racking, there were absolutely no symptoms relating to the bleeding whatsoever! No pain or even fatigue from the blood loss (no signs of anemia). Even more so, he continues to explain, the bleeding always stops spontaneously.

On further exploration of Mr. Smith’s past medical history, he indicates that there is no account of any of his family ever suffering from cancer. He does not smoke tobacco nor drink alcohol beverages. As he views the rancher lifestyle as an ultimate challenge in finding the harmony between physical and mental strength, he is against all forms of illegal drug use. (Medical and social history: no smoking, no drinking, no illegal drug use, and no history of cancer in family)

The doctor carefully records Mr. Smith’s medical history and social tendencies while he prepares the next line of questions to narrow down the differential diagnosis.

Next, the doctor inquires of Mr. Smith’s diet, as his primary complaint is mainly related to digestion and bowel movement. The type of food he consumes can have a direct correlation to his symptoms. As expected by the doctor, Mr. Smith appears to be a big fan of red meat. He fondly tells the doctor that he enjoys barbecue ribs, steaks, and the good old Australian style beef stew. He softly mumbles that he pretends not to hear his wife when she often insists that he needs to add some broccoli and fruits to his meal even though he knows she is right.

Given Mr. Smith’s age, his necessity to strain while using the bathroom coupled with his current diet of high fat/low fiber foods, the doctor has a strong suspicion that he suffers from chronic constipation which may have led to a relatively common condition termed diverticulosis. The doctor is certain that Mr. Smith has this condition secondary to his poor diet over a long period of time. While some individuals may never develop any clinical symptoms from diverticulosis, this condition is extremely common in individuals over the age of forty with increased incidence as the person ages.

The doctor shares with Mr. Smith that diverticulosis is a condition where individuals have formations of many diverticula (many small outpouching along large intestine) located mainly in the S-shaped part of the large intestine leading into the rectum called the sigmoid colon. The doctor continues to explain to Mr. Smith that it is believed that these diverticula form as a result of straining during bowel movement. With increase of the pressure in the colon from straining in an attempt to remove hard stool, the out pouch of the mucosa membrane form as the high pressure pushes through the weak part (the area where blood vessels pass) of the muscle wall of the colon forming many diverticula in the process. The doctor continues to explain that the painless bleeding that Mr. Smith experiences is most certainly due to the many newly formed diverticula breaking the small blood vessels as they herniate through the colon muscular wall.

Since one of Mr. Smith’s symptom is rectal bleeding, the doctor wants to rule out any possibility of colon cancer. The screening of choice is colonoscopy. This test allows the doctor to visualize Mr. Smith’s the large intestine through a small camera searching for any potential malignant mass. The doctor smiles as he informs Mr. Smith that he is glad Mr. Smith will be undergoing this particular examination since it has been long overdue for him. It is recommended that colonoscopy examination should be perform every ten years starting at the age of fifty to monitor the intestine’s wellbeing.

The result of Mr. Smith’s colonoscopy confirms the doctor’s suspicion. The exam distinctly ruled out malignancy of the bowel while showing many diverticula throughout the sigmoid section of the intestine confirming the diagnosis of diverticulosis.

The doctor shares with Mr. Smith of the importance of incorporating fiber into his diet. As the best treatment/prevention of diverticulosis is to avoid constipation, a diet high in fiber is recommended as fiber bulks up the mass of the stool allowing it to move through the colon with fluidity without exerting unnecessary pressure through straining, effectively decreasing the of risk of diverticulosis and other conditions such as hemorrhoid that are associated with constipation. Along with dietary fiber, staying hydrated, and regular exercise will help maintain a healthy bowel while preventing further complications which can arise from diverticulosis.
